Instructions for Keeping Food and Activity Records

To obtain hard copy of Food or Activity Records: In the DINE Healthy software, select Foods (or Activities) from the Main menu bar and click Print Record Form.

  1. Write CLEARLY and RECORD immediately after eating or participating in physical activity.
  2. Record each meal/snack in the breakfast, lunch, dinner, or snack sections immediately after it is eaten AND record each physical activity or exercise in the flexibility, strength or cardiovascular endurance sections.
  3. Complete ALL COLUMNS for each meal / snack and physical activity / exercise.
  4. Record each food or activity on a SEPARATE LINE.
  5. Use the back of the record for ADDITIONAL FOOD / ACTIVITY ITEMS if needed.
  6. Start EACH NEW DAY on a NEW SHEET.
